Although we take every precaution to minimise all risks to our staff, we understand that sometimes further resources need to be accessed under our Duty Of Care to best support those in our care. To do this, we ask that the following form be completed.  

Payment will cover your food and transport.

* In the event of an accident or illness I authorise the Event Organiser/s to seek appropriate medical treatment, and consent (where it is impractical to communicate with me) for me/my child to receive any x-ray examination, medical, surgical or hospital treatment as may be deemed necessary by a licensed physician and/or surgeon.

I agree to meet the expense of me/my child for any ambulance or other emergency transportation costs that may be required.

I agree to meet the expense of me/my child being returned home to my home, by the director or leaders accompanying me/our child and then rejoining the group, due to illness or injury.

I understand that such an arrangement may be necessary due to illness, injury, or if, in the opinion of the Event Organiser/s, non-cooperation of any description or the inability to meet the rigorous requirements of the activity by me/my child.

*Many of our activities are strenuous and can be challenging for those with health complications. If you get tightening around the chest or difficulty breathing or any symptoms of Long Term COVID-19 please reconsider if this activity is for you.
