I have been informed of the nature of the activity and understand that there may be an element of risk involved. I agree to be responsible for taking the time to learn safety techniques and the proper use and limitations of the equipment I will be using. I acknowledge I may refuse to participate in any part of the activity I feel apprehensive about, (if this does not endanger myself or the other participants and leaders). I acknowledge that any snow sports are organised by individuals at their own choice and own risk, referred to Perisher Ski Resort for more information about their safety procedures, expectations and agreements.
I understand that skiing, snow-boarding or tobogganing is not part of the organised weekend and this may occur as individuals take the opportunity to do so on “own risk” basis, if I choose to do this, I accept it is entirely my choice and responsibility and that as an adult I sign to agree with these terms and conditions.
I agree that if I suffer injury or illness, the organisers can arrange medical treatment and emergency evacuation services, as the organisers deem necessary for my safety or well-being.
I am aware, in signing this document, of the risks of the above-named activity and am willing to accept this risk and agree to release, to the full extent permitted by law, AUSTRALASIAN CONFERENCE ASSOCIATION LIMITED (ACN 000 003 930) and/or SEVENTH-DAY ADVENTIST CHURCH (GREATER SYDNEY CONFERENCE, VICTORIAN, SOUTH AUSTRALIAN, NORTH NEW SOUTH WALES, WESTERN AUSTRALIA, NORTHERN AUSTRALIA, SOUTH QUEENSLAND, TASMANIA & SOUTH NEW SOUTH WALES) LIMITED and its employees and agents from responsibility for any injuries which I may suffer as a result of participation in this activity.
In the event of accident or illness, I also authorise the Activity Director to consent, where it is impractical or communicate with me, for me to receive any x-ray examination, anaesthetic, medical, surgical or hospital treatment as may be deemed necessary by a licensed physician and/or surgeon. I also authorise to engage such treatment. I agree to pay the appropriate fees for such and any ambulance or other emergency transportation costs, which may be required.
I understand and accept responsibility for all of my choices and actions and will choose accordingly in the understanding of the standards of the SDA Church and its codes of conduct. I agree that by direction of the organiser/event manager, my ticket may be revoked without refund for reasons of offensive, non-compliant or discriminant behaviour and will demonstrate/show/enact attitudes/actions of respect and compliance with the directions of the event manager/coordinator and/or staff. I understand that such an arrangement may be necessary due to illness, injury, or if, in the opinion of the Activity Director, non-cooperation of any description or the inability to meet the rigours and requirements of the activity/camp or process.
I consent to release all photos and videos for the Australian Union Conference Youth Departments and its affiliate’s promotions and use. I understand that these photos may be used for various Conference Youth Websites, publications, or the Seventh-day Adventist Church-associated organisation’s advertising purposes.
I agree to attend this event on this understanding.
